Abstract:Foram dissecados 40 antebraços de cadáveres humanos. Em 12,5(por cento) 5 casos, foi encontrada a anastomose de MARTIN-GRUBER (A. MG), sendo 2 no lado direito e 3 no esquerdo. Do total de 5 casos, duas destas anastomoses ocorreram entre os ramos destinados ao músculo flexor profundo dos dedos, uma do ramo do nervo interósseo anterior e duas diretas do tronco do nervo mediano para o nervo ulnar. Abstract:Ulnar neurolysis and transposition operations were done on twenty-three patients with leprous neuritis. The pain was relieved in all cases and motor recovery of various degrees was evident. Follow-up of up two years showed that most of the motor recovery occurred between six and twelve months after the operation. The extent of damage and recovery were related to the type and durations of neuritic involvement. (AU).

Abstract:The arteriographic pattern of left hand vessels was studied in 20 patients in leprosy by percutaneous brachial arteriography. Arteriographic abnormalities noted consisted of occlusion, narrowing, tortuosity, dilatation, irregularity and incomplete filling of the lumen by contrast medium. Such abnormal findings were seen in all the arteriograms studied and more than one vessel involvement was noted in over 50 percent cases. This study clearly demonstrated that arterial involvement in leprosy was frequent. No correlation was found between motor weakness and vascular abnormalities. There was no difference in arterial lesions between the patients with and without trophic changes. There was also no correlation between the severity of vascular changes and decline in motor nerve conduction. Degree of histopathological abnormalities in the sural nerve biopsy from these patients showed no features of micro angiopathic neuropathy. It is concluded that the observed vascular abnormalities do not contribute significantly in the genesis of neurological deficit in leprosy.(AU).

Abstract:Although neuropathy is the most striking feature in leprosy, the effects of various drugs are usually assessed by dermatological and bacteriological improvement. This short term electrophysiological study shows that both antileprosy drugs, DDS and Sulforthomidine, do not affect the insidious deterioration of motor unit condition in lepromatous and borderline BT patients. Differences observed in the results of the two groups were discussed.(AU).

Abstract:Motor and sensory nerve conduction studies were performed in the distal part of the ulnar, median and radial nerves of 12 tuberculoid and 12 lepromatous leprosy patients, compared with 15 normal subjects. Slowing of sensory conduction velocity (SCV) was shown in all nerves with no difference between tuberculoid and lepromatous patients. The radial SCV slowing is correlated (P less than 0.001) with the clinical findings. Impairment of motor distal latencies was observed only in tuberculpoid patients. It is concluded that the radial SCV is the most reliable conduction test and is proposed as an early diagnostic test for leprosy.(AU).

Abstract:Leprosy bacilli of human origin were inoculated into a white-handed gibbon by the i.v. and i.p. routes, and also locally into ears, testis and around an ulnar nerve. The animal was observed closely during a period of nearly 15 years and did not exhibit any clinical evidence of cutaneous or neurological disease. At death, a wide range of tissues was taken for bacterial counts and histological examination, and a disseminated and progressive infection was demonstrated. Acid-fast bacilli were found in many sites; their morphological appearance, distribution in nerves, and pattern of multiplication in mouse foot-pads, and also the presence of anti-mycobacterial antibody in the serum and the absence of specific lymphocyte transformation were all in keeping with an infection by Mycobacterium leprae, at an early lepromatous infection in a primate. The findings are discussed in relation to the long incubation period of lepromatous leprosy and the difficulties of diagnosing the disease at an early stage in man.(AU).

Abstract:Peripheral nerve trunks were examined from a case of lepromatous leprosy. M. leprae was seen in the nerves in several types of cells. isolated nerve fibre preparations showed demyelinating lesions due to Schwann cell damage and complete nerve fibre degeneration. The causes of this mixed pattern of damage may involve both a direct effect of the bacilli on nerve fibres and various epiphenomena.(AU).
